Insurance Information and Pricing
If you have BCBS PPO, Anthem PPO, Cigna PPO, Humana PPO, or participate with PNOA or Multiplan (typically indicated by a logo on your insurance card), you may be eligible for for full coverage of your lactation services with me through The Lactation Network (TLN). Please click on this link to verify coverage: https://go.lactationnetwork.com/JocelynShulerLLC. When seeking approval through TLN please text, call, or email me with your name so I know to look out for you, as TLN will not pass this information along until you are approved.
If you have any other insurance plan or do not wish to seek approval from TLN, you will be billed as a self-pay client ($240) on the day of our visit. I will provide you with a Superbill.
If you have a Maryland Medicaid card, you will receive a $100 discount off my self-pay fee, bringing your visit charge to $140.
There is no travel fee for a home visit anywhere in Montgomery County, Maryland. If you reside outside of Montgomery County, Maryland, or prefer to be seen in an office environment, I encourage you to make an appointment at my office in Rockville, Maryland.
